چکیده

Whether acute hematogenous dissemination of tubercle bacilli results in miliary tuberculosis, tuberculous meningitis, or a combination of the two, the ensuing diseases, though very closely related etiologically, are nevertheless distinct clinical entities, each with a different prognosis and therapy. The immediate mechanism, which is responsible for the infection assuming one or the other clinical form, is a matter of speculation and may be due to a variety of causes such as: the anatomical position of the primary source of dissemination, the amount of infected material thrown into the circulation, the frequency of disseminations, possibly the vaguely understood individual variation in organ susceptibility, the patients’ general resistance, or a combination of these and other causes. An answer to this question, which may supply the key to prevention and, perhaps, to more successful therapy, must await further study. The reported results of treatment of acute disseminated tuberculosis with streptomycln have varied greatly. Only certain types of miliary disease (probably of a predominant local pulmonary type) have responded with any degree of permanent success.

Tubercles of the choroid.

In 1948 two of us (Illingworth and Wright) reviewed the literature conceming tubercles of the choroid and described the natural history of the disease as seen in children under our care. We pointed out that the favourable response of children with miliary or meningeal tuberculosis to streptomycin treatment had for the first time made possible the study of the natural history of choroidal tuberc...

The effect of streptomycin on the lesions of tuberculous meningitis.

The extensive therapeutic use of streptomycin in tuberculous meningitis has necessitated the study of the effects of this antibiotic on the pathological processes of this disease. The conclusions of investigators concerning the alterations in the pathological picture in treated cases vary. Smith, Vollum and Cairns’ found prolongation of the clinical course of the meningitis, but several of thei...

CHILDHOOD RESPIRATORY TUBERCULOSIS IN IRAN

To estimate the bacterio-epidemiological situation of childhood respiratory tuberculosis in Iran, 2185 symptomatic patients of 0-14 years old were studied. Among 110 bacteriologically confirmed cases, 60% were less than five years old and the number of females was slightly more than males. In 21 % of patients, the Mantoux test was negative. None of the miliary cases had a history of BCG va...

Current results in treatment of tuberculous meningitis and miliary tuberculosis.

The purpose of this paper is to present the results of treatment of tuberculous meningitis in children who were admitted to the Children's Hospital, Sheffield, during 1954. Fifteen of these were admitted to the professorial unit, while five were under the care of Dr. T. Colver. The study is a direct continuation of earlier work (Illingworth and Lorber, 1951 ; Lorber, 1951; Lorber, 1954a, 1954b)...
